Learners are encouraged to reach out to the Learner Experience Unit by submitting an Online Disclosure Form.
The Learner Experience Unit will strive to contact you within 3 business days of receipt of a submission to book a confidential intake meeting.
You have the option to submit a Disclosure Form anonymously (i.e. without the requirement that you provide your identity), with the understanding that doing so is subject to certain limitations. Note that a UTORid is still required to access the form, even if one wishes to submit their concern anonymously. For more details, please see Confidentiality and Anonymous or De-Identified Reports.
Note that if you submit your name and email, a meeting is required with a case manager in the LEU prior to escalating a report, to review the process and obtain your informed consent to escalate. If you do not wish to meet with the LEU, you can submit your report anonymously or return at a later time when you might be open to a meeting.

Although we recommend using the Learner Experience Unit to discuss, disclose, or report mistreatment, we recognize that some learners may convey information regarding experienced or witnessed mistreatment on course and teacher evaluation forms. While every effort is made to review evaluation forms in a timely manner, you should not assume that course or teacher evaluations will result in action in the same manner as a report of learner mistreatment through our office.
