External Teaching Awards Nomination Form

For these awards, nominations are first adjudicated at the Faculty level to select the candidate that will move forward as the official Temerty nominee:

• The first stage of the adjudication requires a simplified version of the official nomination package. You can preview the nomination form here.
• If chosen as the official Temerty nominee, the Office of the Vice Dean, Medical Education will support the nominee to develop their full nomination package by the external deadline.
• Please direct any questions to medicine.awards@utoronto.ca rather than to the organization offering the award.

Tips for completing the form:

• Once started, it is not possible to save your progress and return to the form later.
• To avoid losing work in the event of technical issues, we recommend that you draft your responses in Word and paste them into the online form.
